Nutraceutical News
I discovered a rather intriguing perspective on the world of wellbeing, health and nutrition in the latest issue of the journal World Review of Science, Technology and Sustainable Development (2008, 5, 104-123). In it, Sundus Tewfik of the Department of Health and Human Sciences at London Metropolitan University and Ihab Tewfik of the University of Westminster, shed light on nutraceuticals, or functional foods as they are sometimes called. You will doubtless have seen mention of functional foods and botanical dietary supplements as they seem to fill the advertising space in Sunday supplements and feature regularly in lifestyle magazines.
Apparently, nutraceuticals promote wellbeing and underpin public health by providing a supposedly natural way to lower raised cholesterol levels, help unblock clogged arteries, ward off otherwise inevitable cancers, and ease the machinations of the over-sensitive gut. All this, without anyone having to resort to pharmaceutical products and double-blind placebo-controlled clinical trials.
It’s the opening paragraphs of the paper that were the most interesting with regard to the state of play when it comes to the gradual public acceptance of the marketing hype surrounding nutraceuticals:
Consider this domestic scene: it is a typical Sunday morning in an English household in the city of Westminster, London. Mrs Jones is preparing breakfast for her family. Like most mothers, she is concerned about her family’s nutritional status and tries to cook healthy meals. This morning it is an English breakfast, but not just an ordinary one.
Personally, I doubt there are many such domestic goddesses around these days, particularly in Westminster, but more to the point, I think many families these days rush breakfast with at best a quick splash of synthetic fruit juice, and some artificially flavoured cereal rather than feasting on the great English breakfast. But, that aside, the researchers then describe the menu:
The wholemeal bread was made out of grains to increase dietary fibre intake and essential micronutrients, thus helping bowl [sic] movement and support the gastrointestinal tract. The omega 3-enriched eggs will enhance the immune system, reduce the risk of cardiovascular diseases (CVD) and blood clotting. The sugar-free orange juice has added vitamins and antioxidant nutrients, believed to reduce the risk of diabetes, CVD and cancer. The extra virgin olive oil she uses to fry the eggs [You shouldn't use extra virgin to fry, it degrades rapidly at high temperatures, db] has been chosen to help lowering her mother-in-law high cholesterol.
Agreed, wholemeal bread is probably better for bowel movements than bland and bleached white bread, but wholemeal, while functional, is not the breakthrough health product. After all, my grandmother extolled the virtues of roughage to me decades ago.
The mention of omega acids and antioxidants is possibly valid, but there are no wide-scale trials yet to backup some of the wilder claims made in the popular press. Indeed, adding to one’s diet excessive amounts of antioxidants could

compromise one’s
immune systemantioxidants could ultimately compromise one’s immune system. The i immune system, after all, relies on its own oxidants to kill invading pathogens and destroy cancer cells.
Next, the team suggests that the breakfast sausages with “less than 1% fat” will somehow eliminate any risk of CVD posed by saturated fatty acids. Well, 1% might be described as low fat, but I’d prefer the term reduced, but again, I am not sure how functional are reduced-fat sausages.
One of my many pet peeves regards the claims surrounding so-called organic foods. The jury is still well and truly out on whether there are any benefits and as for the lack of pesticides and fertilisers requires some of those used by organic practitioners are already known to be more hazardous. The beans, tomatoes and mushrooms being “organically grown” also does not take into account the fact that just because Mrs Jones in Westminster can afford the luxury of organic this does not mean organic is better for the world. Energy expenditure for organic farming on a large enough scale to feed the world could be significantly greater than in non-organic methods.
Finally, the salt used by hubby was specially manufactured to help minimise his high blood pressure. Well, yes, I’d concede that’s a functional food. But, whether or not a sprinkle of non-sodium salt is going to benefit Mr Jones’ blood pressure is not beyond doubt; alcohol consumed, cigarettes smoked, processed foods eaten, and genes inherited, play a much bigger role. More to the point, given that the sausages will have been made with salt, why not simply not use salt at all, those organic foods are claimed as more flavoursome anyway, so no need to enhance with salt.
The researchers end their introduction with the thought that this Westminster breakfast is not a scene from ‘Balanced-Nutrition’ program on national television, it is the era of medicinal and functional foods and it is happening as we read this paper in many parts of the world. This is not just food this is ‘functional foods’.
Well, I am not so sure, most of what they describe is not functional in the conventional sense, although elsewhere in the paper they list dozens of functional foods and herbal supplements such as ginseng and Gingko biloba. There may certainl dozens, if not hundreds, of food products now on the market that claim some kind of health functionality. But, the whole notion of a supplemented diet that might improve wellbeing has been stacked very high in recent years. There are shelves full of milky probiotic drinks full of microbes that supposedly repopulate your intestine with good bacteria, products with plant steroids to reduce cholesterol, ward off the menopause, and dozens of herbal extracts each one of which is seemingly a cure-all for a wide range of disparate health conditions.
As long ago as 2001, uber-skeptic of the alternative medicine movement, Edzard Ernst of Exeter University, asked whether functional foods, neutraceuticals, and designer foods are simply

He pointed out that so-called functional foods invariably contain less than therapeutic quantities of their active ingredients and may contain higher levels of apparently “unhealthy” ingredients such as saturated fats.
Almost a decade later, there is still a lot of health hype in those lifestyle magazines and supermarket shelves are increasingly stacked with organic produce, with its premium price tag, and healthstores are packed with botanical products from all corners of the globe. Is this food fad just a cynical marketing exercise, not only for food manufacturers, who can charge more by making dubious health claims for their products, but also for the pharmaceutical and health-care product companies who are now, as blockbuster pipelines dry up, providing the ingredients for the functional diets we are all being told we must consider.
I suspect, once the advertising revenues dwindle and the lifestyle magazines become necessarily bored with the functional food fad, that ultimately many will be left on the shelf while the next moneyspinner rings the changes at the checkout.
UPDATE> I’ve been having an interesting correspondence with commentator David Lustig who points out that there are some very rigorous double-blind placebo-controlled trials omega 3 products. These were carried out for the approved prescription drug Omacor, sold by Reliant, which is essentially nothing more than purified, concentrated fish oil. It has a profound effect on lowering triglycerides and is currently one, if not the only effective approved drugs for hypertriglyceridemia. Lustig suspects it will probably sell at least US$500M this year.
It’s an interesting point at the extremes there is a blurred division between the pharma and the nutra. However, this FDA approved product can in no way be categorised as being of the same ilk as probiotic yoghurt, although it is nothing more than concentrated fish oil a lot can happen when something is concentrated. More to the point, it will be almost 100% free of the kinds of contaminants, such as mercury, that might be found in the healthfood store kind of omega 3 fish oil products that are at much, much lower concentration.

I believe that they are the next big thing and I confess that at this stage it is a belief and not a fact; simply because there is an insufficiency of facts to make the arguments conclusive.
[As part of my consultancy work, I do track consumer trends in the foods and nutrition segment; amongst other things identifying the behavioural shifts that will impact consumer flavour preferences in 2018 etc.]
For me “Health” has moved and is moving forward in two different directions, what I describe as “Fashionable Health” and “Functional Health”.
Fashionable Health is accompanied by all the high tech home gym equipment, smart textiles, health clubs, smart restaurants and attracts a large number of consumers who not just want to BE fit and healthy but want to be SEEN as being fit and healthy.
Functional health is for me associated with the smart fruits, Omega 3’s, vitamins, 5-a-day fruit & veg trends, healthy and tasty foods, healthier eating and “do-good” foods, etc.
Nutraceuticals are but one part of the Functional Health foods development - look also at the “good for you” fruits like acai etc.
The fact is that science has developed to give us a better understanding of why more exotic fruits and vegetables may be as good for us over-processed food consuming Westerners as they are for the locals who consume these foods in their native growing habitat.
I agree with the gist of your article that there is as yet (and possibly will never be) a fully controlled scientific study that takes balanced samples of the population and feeds one non-organic food and the other organic food and then measures their respective levels of “health”. In essence I hope we never allow an experiment like that to happen as it will be forcing people to modify their dietary habits in a vain attempt to pinpoint the proverbial needle-in-a-haystack.
That said, despite being relatively cynical of marketing hype (as a former Marketing exec this is a case of poacher turned gamekeeper) my household is converting to organic (without running blind taste tests), is purchasing high Omega-3 containing foods (and silently pray that we are getting smarter!) and to mention a brand-name that seems to be getting right, consuming Danone’s Actimel as part of an overall healthier eating trend.
We’ve also just planted an orchard to grow our own fruits organically!
So in summary it’s the next BIG THING!

It’s old news repackaged as the next big thing. Basically, companies are trying to figure out ways to reproduce stuff that’s been around for thousands of years in nature and then sell it for more money. An example are the prebiotics (nutrients that help “good” gut bacteria grow) now being added to infant formula. Not suprisingly the prebiotic formula comes with a premium price and is still not as good as old fashioned breast milk. Unfortunately, adding a nutraceutical to your diet is not going to fix the biggest problem which is just eating too much in the first place.

Hamish, when it comes to medicine and health, a gut feeling is not necessarily the best way forward. Without evidence there are no definitive answers. Moreover, there is absolutely no reason to believe that so-called organic food is any better than non-organic and indeed the opposite may be true in some cases. Look at the case of raw versus steamed carrots as an example how the “natural” state may not be the best. This root vegetable, lightly steamed, allows us to absorb its vitamins much more effectively than eating it raw.
As to the supposed superfoods, again, show me the evidence that a daily handful of blueberries or some exotic fruit that has to be shipped from the tropics to my local supermarket is any better for me than the old “apple-a-day”. Just because chemical analysis reveals that a specific fruit has high levels of antioxidants does not make it any necessarily beneficial. First, one has to ask are those antioxidants actually available to me through absorption if I eat said fruit and secondly, there could be detrimental effects on health, re a kind of modulation of the immune system, for people ingesting very large amounts of antioxidants.

I think the things that will determine whether nutraceuticals are quackery or helpful are:
1) how much science backs up the claims for the various products and how far beyond the science the claims go. There is lots of good science on things like Vitamin D etc., but then you also get totally unscientific claims such as it (any product) is better because it’s natural, or natural products do not have side effects (what about the studies on high levels of Vitamin A increasing fracture risk)
2) how well regulated the products are (not necessarily legislated regulation), as to their production and quality control. It is my understanding that there a great number of herbal remedies and dietary supplements that are extremely variable in their contents from batch to batch.
If I can know that what I am taking this week is the same as what I took last week and that when 10 other people took the same thing they saw specific measurable improvements (not just they felt better) then I believe it will be the next big thing.
If I am told that this product made by some secret process handed down over the generations by medicine men and it will add hair to my chest, stop me from getting colds and flu and clear up my arthritis and … but that there are no drug trials because big pharma don’t want me to know about it then I’m going to run the other way.
There is definitely plenty of solid scientific evidence for various nutritional based products - for example Vitamin D, however.

In response to your remarks, actually when it comes to healthy eating and living, a good “gut feeling” is actually about as good as it gets! In supporting some dimensions of neutraceuticals, my original comment was neither precise nor indeed supported by citing numerous scientific articles. However think about the press coverage and medical studies on 5-A-Day fruits & vegetables, fish oils, daily sensible use of vitamins (as opposed to excessive over-consumption).
Regarding vitamins, we wouldn’t consume them today if scientists of the 18th, 19th and 20th centuries had not concluded that they were the “Next Big Thing”of the time and as a consequence we would still be suffering from scurvy, rickets and other vitamin deficiency associated diseases. The antioxidant effects of the flavonoids in green tea are well proven and their efficacy established in controlled studies for limited applications e.g. anti-bacterial and skin anti-oxidant effects - these may be topical applications as opposed to consumption based benefits (after all not much really survives the acidic environment of our digestive tract).
I am aware of ongoing developments of natural sourced drugs that can bring proven benefits against the common cold, however the herbs taste so foul that to make them available in a digestible format is a huge hurdle to cross. Equally fish oils tend to taste fishy and soya milk based products have astringent taste profiles that again make them really only for the hair-shirt wearing health purists.
These functional foods (as opposed to the quackery pushed “magic pills”) are supported by relevant scientifically validated data - this is why I reiterate that I believe they are the next big thing but appropriate caution and the development of appropriate taste and texture masking agents will be required before they become mainstream.
The points made above on Quality Assurance and consistent functional content are well made and extremely valid; this is why we should all hope that it is big business and not cash strapped entrepreneurs who become responsible for the safe sourcing and supply of these ingredients to us. Not that I am a universal supporter of big corporates, however at least they have the legitimate good sense to fear class action lawsuits and have the financial strength to pay suitable penalties if they aimed to exploit the fad and the health fears.
Finally a response to the point on pre-biotics and pro-biotics - yes there is no better substitute than mother’s milk for babies, totally agree on this point. However I am 43 and my mother is 78, if the pro-biotics are shown to be good for continued gut health beyond the point of being an infant, then surely there is a lot of common sense in making those good gut bacteria available to a wider section of the population????

Thanks for your continued comments Hamish, very interesting. It’s funny you should mention how bitter tasting and unpalatable herbal remedies can be, that’s part of the reason the pharmaceutical industry emerged in the nineteenth century as it took naturally sourced remedies and made them into pills. Indeed, there are few drugs in common use that don’t have an original natural source from aspirin (willow tree sap) to the anticancer drug Taxol from the yew tree and dozens of compounds of marine origin in cancer trials.
I think you may be right that a selection of the nutraceuticals being investigated today will turn out to be the “vitamins” of the twenty-first century. However, I think there is so much marketing hype misinformation about many more that it is becoming a real problem with consumers exploited again and again. For instance, in being sold overpriced so-called organic superfruits and low concentration extracts from trees with odd-shaped leaves etc.
The probiotic milk concept, is yet to be proven. Because these products are sold as foodstuffs rather than medicines the manufacturers can get away with minimal supporting evidence (likewise the cosmetic and antiaging creams companies). They need only demonstrate marginal efficacy in a handful of volunteers neither controlled nor blinded and can effectively say what they like (under current law). If we are to see advances, and the emergence of this century’s vitamins, we need to bring nutraceuticals with medicinal claims into the true medical fold, enforce regulations, proper trading standards and advertising controls. Then the quackery will quickly fall into disrepute and we will be left with the few products that confer a genuine health advantage.

I think the whole nutraceutical thing is a cynical marketing ploy. All foods are functional foods. You need to eat to stay alive. We evolved before we created the food industry so it is not surprising that we thrive on unprocessed food. All the food industry should really do is try not to damage the nutritional value of the stuff it is handling. Employ a few more scientists and a lot fewer marketing guys and things will be fine.

Cynical Marketing ploy or Science Reinforcing observation of Mother Nature?
“Marigold is a true folk remedy with a long history of use, probably back to ancient times. It continues to be used to this day and is approved by the German commission as an antiseptic and to heal wounds. Dukes Ethnobotanical reference lists it as a pain killer (analgesic). It is widely accepted as a treatment for nappy rash. Some folk remedies are simply twaddle. Others have never been properly investigated. But there are some that have been investigated scientifically to be truly effective, and marigold is one of them. Marigold has been the subject of extensive research and has been shown to have anti-inflammatory properties. It is even known in some detail how it might work. Pentacyclic triterpene trihydroxyalcohols, flavonoids, and saponins have been isolated from it, and these are believed to stimulate granulation and increase glycoprotein and collagen metabolism at wound sites. It also shows in vitro antimicrobial and immune-modulating properties.”
What’s the difference between the above statement and what I posted earlier - Scientific Marketing or what?

At the end of the day, as it concerns Nutraceuticals, I believe that people interested in supplementing their diet should consult a Naturopathic Doctor.
My Naturopath was able to easily cut through the “bull” and give me proper direction in this area. A mulivitamin, some probiotics and some ground flax seed daily in addition to a few diet changes have all been a boon to my health.
Just as Family Physicians are experts in pharmaceuticals, Naturopaths are experts in Nutraceuticals.
I highly recommend this type of consult for anyone.

In my mind the science seems potentially solid, but the telling indicator of of quackery would likely be the business model.
If it’s an MLM(multi-level marketing) or in slang “pyramid scheme” then very frequently you’re purchasing “snake oil”. If the product is produced and sold through traditional means by a reputable nutritional supplement company like EAS for example, the product quality would increase significantly.

In my view the dietary supplement/nutraceutical sector is now a mature industry. The Dietary Supplement Health Education Act (DSHEA) was signed into law by President Clinton in 1994. As a result of this legislation, FDA promulgated regulations on labeling, Good Manufacturing Practices and Adverse Event Reporting and has issued guidelines on claim substantiation and other important topics. The NIH has established National Center for Complementary and Alternative Medicine (NCCAM) and provides grant money to study nutraceuticals and other alternative health modalities. A number of universities also have programs that contribute to the body of knowledge on vitamins, minerals and herbs.
The dietary supplement industry has experienced business cycles typical of growing and maturing sectors. Sales soared for a number of years in the 1990’s. Sales leveled off and the industry experienced a great deal of consolidation. The maturity of the industry can also be seen in the recent initiative by the Council for Responsible Nutrition (CRN) and the National Advertising Division of the Council of Better Business Bureaus (NAD) to create a pathway for responsible industry members to challenge advertising that does accurately reflect the state of the science in the area and therefore does not comply with FDA (and FTC) regulations.

I don’t think you can lump it all together. Sure, some of it is quackery, but a lot of it has good research behind it. Just remember that you can’t prove a negative. When you see an article in the news that says “Vitamin C does not help flu symptoms” you have to realize that you can never prove that something does not work. There is a lot of disinformation, probably from big pharma, trying to get people to not use alternative medicine. Nutrition is an elective class in Med School, it is not a requirement.

The two options in your question aren’t mutually exclusive. Nutraceuticals can indeed be both The Next Great Thing and quackery at the same time.
With good marketing, you can sell anything. The Next Great Thing.
Unfortunately, much of that world is based on unproven claims. Anecdotal stories are not a substitute for double blind, placebo controlled clinical trials. The problem is that trials like these cost an enormous amount of money, which is problematic when the agent being tested is a low priced commodity.
There are certainly some success stories. Niacin, also known as vitamin B, does indeed raise HDL - the good cholesterol. But because it has the problem of facial flushing (itchy redness) as a side effect at the efficacious dose for many people, it’s not easy to just go to the drug store and buy a substitute. Pharmaceutical companies have come up with solutions to the flushing issue and are selling them as proprietary prescription drugs. The ability to make money off of it allowed for the clinical trials to prove that it does indeed raise HDL (though no one has yet done an outcomes trial to see if that does actually increase longevity or decrease cardiovascular events).
Anecdotally, though with some clinical trial results, Vitamin C in doses of 2000 - 3000 mg a day will also raise HDL. But with Vitamin C, there are no issues to solve with products that aren’t already available at the grocery store, so it’s unlikely that rigorous trials will ever be conducted unless it’s in combination with another drug like a statin for a combo pill that lowers LDL and raises HDL.
One could argue that clinical trials should be sponsored by the government as a matter of public health. Though where does that leave the nutraceutical business? At best, each company will compete against all others as a commodity (imagine a drug with hundreds of generics on the market).
So that comes full circle back to marketing. Nutraceuticals will sell well for companies that have effective marketing campaigns, regardless of whether there is a true health benefit or not..

Is there room for the category to end up somewhere in between?
I was surprised at the myriad routes to discovery of prospective commercially viable active pharmaceutical ingredients, a powerful lot of which seem to derive from natural sources, many of which are reckoned to constitute foods in some parts of the world.
The mechanistic infrastructure of scientific observation and the posting of models to represent behavior depends on observation of phenomena and the hypothesis of explanatory theory, as I understand it.
When the consumption of ephedra killed enough people, folks began to take that one pretty darned seriously.
A good deal of what people eat, breath or absorb through their skins seem to end up in the liver, the body’s filter, at one point or another according to my very rudimentary understanding of anatomy.
What those substances conspire to do together there and in the bloodstream can sometimes get kind of difficult to predict, as there seems to be a powerful amount of variation in what different people will do, eat and drink at different times.
There’s a Chinese sensibility that all food has medicinal properties, some yang (or positive, termed “heating”) and others yin, believed to be “cooling.” The traditional “five elements” school of medicine holds the liver to represent a positive vessel of yang essence, analogous to the element of wood.
Seems to make as plausible a model as any, I reckon.
I think there may be ample opportunity for this area to become both big business and for certain parts of it to end in tears for all concerned, just as energy and transportation and commodities and realty have ebbed and flowed over time.

Most certainly Gerald, I do believe that the answer to my question lies somewhere in between, like you say it’s not clearcut and yes, indeed, hundreds of pharmaceuticals are of herbal, marine, animal, vegetable, and nutritional origin and continue to be so.
